We ask that when calling for an appointment, if you'll be using an insurance carrier, please have your Insurance Group Number and your Member Number available.
We'll also put a credit or debit card on file at time of creating appointment, please review the posted financial policy available online and in office.
After your insurance has processed your claim any remaining balance is the Patient Responsibility. Patient may be notified of their balance responsibility via text/email and/or paper statement for review and if no payment is made within 30 days, the card of file will be charged for remaining balance.
Self Pay Patients will receive a discount at time of service. Payment will be processed at time of service for Immunizations, Tests, and Supplies.
Out of Network Patients are considered self pay at time of service and can file with their insurance if desired
Prior to calling for an appointment time, pre registering will make your information available to our staff when you call for your preffered appointment day and time.
Our new EMR system as of 8/26/2024 allows patients to register and check in online. The policies below are now digitally signed with our new system at or prior to check in. These documents are available for preview but have been replaced with the digital acknowledgements.
We will require a government ID card and your Insurance ID card in the office.
The front office will also require a form of payment on file.
Patients will be asked acknowledge receipt of APC Financial policy and Authorize Auto Payment of patient balance after insurance is processed.
If you have Advantage or Supplement Plans, please also bring your Medicare card.
